Stolen debit card number. They are charging small charges to my bank account!
Federal authorities have been notified.

April 24, 2010. My credit card number was stolen by these clowns. They charged $4.88 for something I never ordered (nor have I ever done business with them). I immediately cancelled that card. When you call the number, 504 273 6187, in Kenner, LA you get a recording saying that they have changed their website from "www.formsplanet.com" to "www.formsplanet.net". The only way that you can contact them is by sending an e-mail to [email protected]: NOT something that I'm going to do. I did report them to the FTC. I also attempted to leave them a message at 504 273-6187 advising them that I had reported them to the FTC (don't know if they got it).
Hopefully enough people report these clowns to the FTC to shut them down permanently. Or maybe I'll just take a ride to Kenner, LA and pay them a personal visit...

What happened to you was exactly what happened to me.  Credit card company fraud division is suppose to be looking into it now.

1 May 2010.  Discovered a $4.88 charge to the debit card linked to my bank's checking account.  Contacted my bank (Bank of America) and they supplied me with the 504- number above.  Called and got the same recording that Mike did (above).  Have never contacted them nor have I ever done business with them.  Don't know how they got my card number.  Called my bank and they reversed the charges, reissued a new debit card, and are pursuing the company for legal action.
