Wrong, all US numbers start with a +1 than the area code. Don't know where you live with a +2 in the number.

He is talking about the prefix - the first three digits of a seven digit phone number.  The first of those digits cannot be a "1".

No, all US number do not start with a one.  Only when you are dialing long distance from a landline.  You certainly do not have to dial "!" within you own area code..
