I received a similar letter in the end of Sept.  I sent in my claim form and received a check about 2 weeks later.

Just thought I would give my input.

I got the same letter but i got my refund already. Is it saying that the information was lost and I could get more money. Im not sure about this. It does sound like a scam to me.

I  received a letter from the same company and it doesn't look legit at all.

Follow this link to get the refund yourself.  Don't let them take a large portion of the money you are owed by assigning your rights to them.

This is a collection agency and just a cute way for you to confirm where you actually live by getting the photo ID. This is probably your drivers license so they wil have you confirming, via the notary, your current address.Bunch of BS
