This is legitimate! The caller is Account Billing Services, as "ABS" states in their comment above. And I knew that I owed Saltzer Medical Group, but I thought that it had not gone beyond 90 days. I called Saltzer directly, talked to Billing Dept, and they straightened me out.

ABS wants a minimum $50/month, and charges 1%/month (12%/year), until balance is paid, which is way better than any credit card. So I thought I had 2 months left before paying, and I will pay them 2% before I get the balance paid off. Not at all unreasonable.
