The call was from Humana Health Insurance call center concerning making an appointment for an annual physical at a Humana operated VA Community Clinic.  The caller left a different call-back number on my voice mail to call to make the appointment.  Rather, I called the clinic I go to and they verified the legitimacy of both numbers.
