...got a call from this number (cell phone) claiming to be from the Colorado Dept of Health and the Environment, supposedly conducting a phone survey on air quality. CDPHE also happens to be the state agency that provides copies of vital records, birth certificates, etc. A few days later, I got a return receipt from CDPHE for something I never sent to them. The call was most likely a pretext for an attempted identity theft.
