Wanting to know kids that have immunization, but then asked too many personal information questions

Same, started asking all sorts of personal questions, I declined to state.  It got to the point I asked to end the interview, because I have no idea what they had to do with immunization records.  Asked city/state, income, and many other questions I refused to answer.

Is this really an immunization questionnaire? They ask a lot of personal questions.
