Received the fax today, although the header date says 6/12/2011 ref no 89394

with the removal number listed as 1-855-845-2761 which is busy, removal website listed as www.dataremoval.org which is mostly blank and half in German- looks fake!
I called the 312 number, a nice woman answered "Letisha(?), Healthcare Services" who pretended to take down my fax number and said she would get it removed. Will write back if I receive any more.
Received a digital fax soliciting health insurance today. I found it when I logged into my AT&T unified messaging system. While there is no fax machine attached to my phone number, AT&T records it as it it were another call. There was no sending fax number on the fax (AT&T unified messaging reported the calling number as "identity withheld"). The number to call that was in large type in the body of the fax was 312-651-6070.
