Received a call from this number this evening and when I called the number I got a message that said the number has been changed or disconnected.

Received a call from 540-904-0059 at 3:20 pm on 01-06-2015. Female who did not identify herself by name claimed to be from Carilion Clinic doing a survey of patients for my doctor.  She had my private information and medical information. After three questions related to the doctor, she asked a question that was not related to the doctor.  I terminated the call.  She seemed to b phishing for information.  Be sure you know who you are talking with on the phone.
