Who called from 8014421039

I called this number back from my work phone and it's a recording asking you to enter your 10 digit phone number and take a healthcare survey.  Blocked the number.

Missed a call. No message left. I believe it's IHC because their phone numbers are all 801-442-1000's, but I hate when​ they won't leave a message.

Did any of you get the call within a few days of an appointment to an IHC doctor or facility? No one raise your hands. I know. It's embarrassing. It's ihc asking you to take a survey related to your recent visit.
