Sounds like a scam.  Caller claims I filled out a form and requested mortgage insurance information.

Caller became confrontational when I assured him that I did no such thing.

Caller became more aggitated when I asked him not to call my number again.

NBow Caller calls gets the voice mail and hangs up several times a day.
