I received the first call yesterday, which I immediately blocked. They tried to call again a few minutes ago and my call blocker intercepted it. The caller ID name comes across as "Unknown Name".

Have received numerous calls from this number in the past 3 days - just now received 3 in the space of 30 minutes! I've picked up twice and it's an automated survey about the Republican party. There's no option to put your name on the Do Not Call list. I'll be filing an FCC complaint for this number immediately.
Surveys and political calls are exempt from the National Do Not Call list and it's highly unlikely this caller would have an internal list for you to "put your name on". Your best bet is to block the number if you don't want to do their survey.
