It's Park Nicollett Clinic in the Mpls MN area.  The number that appears on the screen is a generic outbound number for them.  They usually leave a VM with instructions for their different call-back number and why they're calling.

Someone from Park Nicollet following up about a chiropractic referral after I went to urgent care for a pain in my shoulder.
