This survey is sponsored by the Centers for Disease Control and Prevention (CDC), a federal agency of the U.S. government, and is conducted by the National Opinion Research Center (NORC) at the University of Chicago.
This important national survey is intended to improve the health of Americans by collecting information about childhood, adolescent, and adult immunizations. The information collected helps identify communities or areas needing additional resources for immunization programs.

I also was receiving calls from them. I started a survey but when I had to get specfic about my children with their ages and names I opted out. They have called everyday sometimes more than once, I have even told them I do NOT want to do this survey (I dont have 20 uninterrupted minutes). I just called this phone number and she assured me I would be removed from the call list. Thanks for posting!
I finally came across this number (866-999-3340) and the woman who answered was very apologetic and told me I would be taken off the list and she was sorry for the inconvenience. Amazingly, I was taken off the list shortly thereafter.
