A lady called and left a message saying it was Access Medical Urgent Care Facility and that I had a balance from August 2012, and January and February of 2013 and needed to give them my updated insurance information or I would be responsible for the payment. She called from several states away, I've never stepped foot inside an Urgent Care, and I didn't go to the doctor at any of those times...
