This important national survey is intended to improve the health of Americans by collecting information about childhood, adolescent, and adult immunizations. The information collected helps identify communities or areas needing additional resources for immunization programs.

Got the same kind of call today. Offering $10 for the information. Said my cellphone number came up on a list. Tried to get information about my family, the land line, and the name and address to send the money. Of course, got nothing but still disturbing!!
Called yesterday, and left a message asking me to call them back to participate in a 10$ survey. Today they called and asked if I had a land-line, and for the # of children between 12months-3years living in my household. Followed by a request for my name and address of where to send the 10$--------I left a address to a vacant building whose mail is forwarded to my home. Lets see if my 10 comes in!
