Purchased this insurance after receiving the fax.  After several routine doctor visits that were not covered and reading the online complaints, I cancelled the insurance.  The name on the so called insurance card was BeechStreet Insurance or National Alliance of Associations.  After several unsuccessful calls to 856-374-1599 I was able to call and talk to a live body at 800-919-5514 who canceled my insurance.

I was told this Insurance was major medical. I got scammed. I'm trying to cancel.
